چکیده

T he past several years have witnessed unprecedented advances in rheumatology, with the introduction of several new compounds capable of halting the relentless progression of joint destruction and functional disability in patients with rheumatoid arthritis (RA). With these successes, however, have come new pressures for medical imaging to resolve even the slightest traces of erosive joint damage and to identify pre-erosive inflammatory features that can accurately predict which patients will go on to severe functional debility if they do not receive aggressive structure modifying treatment immediately.

Ultrasonography detection of early bone erosions in the metacarpophalangeal joints of patients with rheumatoid arthritis.

OBJECTIVE To compare ultrasonography (US) and magnetic resonance imaging (MRI) in their capability to detect bone erosions in early-advanced rheumatoid arthritis, where no erosion was evident on conventional radiography (X-ray). METHODS Metacarpophalangeal (MCP), radiocarpal and ulnocarpal joints of 13 patients with rheumatoid arthritis, with bone erosion that was not detected by conventional...

The usefulness of magnetic resonance imaging of the hand and wrist in very early rheumatoid arthritis

INTRODUCTION Magnetic resonance imaging (MRI) was used to study the hand and wrist in very early rheumatoid arthritis (RA), and the results were compared with early and established disease. METHODS Fifty-seven patients fulfilling the new American College of Rheumatology criteria for RA, 26 with very early RA (VERA), 18 with early RA (ERA), and 13 with established RA (ESTRA), (disease duration...

Early Metacarpal Bone Mineral Density Loss Using Digital X-Ray Radiogrammetry and 3-Tesla Wrist MRI in Established Rheumatoid Arthritis: A Longitudinal One-Year Observational Study

Objectives. Early change in rheumatoid arthritis (RA) is characterised by periarticular osteopenia. We investigated the relationship of early metacarpal digital X-ray radiogrammetry bone mineral density (DXR-BMD) change rate (RC-BMD, mg/cm(2)/month) to longitudinal changes in hand and feet radiographic and wrist MRI scores over 1 year. Materials and Methods. 10 RA patients completed the study a...

Role of Ultrasound and MRI in Detection of Hand and Wrist Joints Erosions in Rheumatoid Arthritis Patients, Comparative Study

Objective: To compare the role of musculoskeletal ultrasonography and MRI in the detection of erosions of the wrist and hand joints in Rheumatoid arthritis patients. Patients and Methods: This study included fifty patients (39 females and 11 males) with rheumatoid arthritis. They were subjected to high resolution grey scale ultrasound study and MRI examination of the clinically dominant wrist j...

[Dynamic contrast-enhanced magnetic resonance imaging of the wrist in early arthritis].

OBJECTIVES MRI has been proposed as the imaging method of choice to evaluate the long-term outcome in patients with early arthritis. The role of dynamic MRI, performed at presentation, in predicting the outcome of patients with early arthritis has been addressed in the present study. METHODS 39 patients with early arthritis, involving at least one wrist, were studied with clinical visits and ...

Magnetic resonance imaging of the wrist in early rheumatoid arthritis reveals progression of erosions despite clinical improvement.

OBJECTIVES To investigate the progression of joint damage in early rheumatoid arthritis (RA) using magnetic resonance imaging (MRI) of the wrist and determine whether this technique can be used to predict prognosis. METHODS An inception cohort of 42 early patients has been followed up prospectively for one year. Gadolinium enhanced MRI scans of the dominant wrist were obtained at baseline and...
